SOUTH SALT LAKE, Utah (ABC4) – A silver alert was issued Tuesday night to a missing woman from South Salt Lake.
Susan Anne Lavender, 57, was last seen Tuesday at around 1:45 p.m. at the Avalon Valley Rehab Center at 2472 South 300 East in South Salt Lake.
She is described as 5 feet 4 inches tall, weighs 150 pounds, has brown eyes, and has short black hair that is combed over.

She was last seen in gray sweatpants.
It is not known where she might be going.
Officials say Lavender has dementia and cognitive communication problems.
According to South Salt Lake Police, lavender is also bipolar and schizophrenic.
